Welcome to Triangle Mental Health Foundation Well-being Questionnaire
Our Self-Assessment is free, and a confidential way to find out about your mental health this is a simple yes or no questionnaire which will help to indicate if you are suffering from stress, anxiety or depression.
The questionnaire is designed to measure the four related negative emotional states of stress, anxiety, depression and life’s events. The assessment tool uses a scale rating system to measure the severity of a range of symptoms related to stress, anxiety, depression and life’s events in effect the questionnaire in a single assessment tool.
Every new client requesting a free appointment will be required to complete the online questionnaire before their appointment. This method enables us to ‘hit the ground running’ rather than spending your first one-to-one session fact-finding, we are already in a position to discuss solutions to move you forward on a path to recovery.
The questionnaire was specially developed by David Briscombe, Co-Founder of Triangle Mental Health Foundation, and has been designed to meet the specific requirements of Triangle Foundation Health Professionals.
David has been a Member of the International Stress Management Association (ISMA) Since 2000 and is uniquely qualified to help you make breakthroughs in your life. David has been writing self-care stress-wise and resilience training programmes for over a quarter of a century and has taught many thousands of people new self-help skills that have attended his unique courses.
Testimonial of his reputation comes from the number of patients, referred to David by hospitals doctors and consultants. Within his private practice, he has treated a wide range of mental health issues, including Post Traumatic Stress Disorder (PTSD) Anxiety, Depression, Sleep and Obsessive-Compulsive Disorders, Myalgic Encephalitis (M.E.), Bulimia, Anorexia Nervosa, Irritable Bowel Syndrome (IBS), chronic pain, and weight reduction.

A senior health professional will then contact you generally within 24 hours, via the Email or phone number you supplied in the questionnaire to arrange your free meeting to discuss the results. (please check your spam and junk mail before you contact us) appointments are available Monday – Friday 10 am-4 pm.
Please note we are not a walk-in centre, and we do not give assessment results over the phone or by Email.

Recognising the symptoms of stress anxiety depression
Everyone feels down sometimes, especially during stressful periods. Feeling low, having difficulty sleeping and lacking motivation are all common responses to the trauma of losing a job and unemployment. If you are worried or unhappy, you should consider seeking support to help you through this challenging period.
Sometimes, these feelings may be indicators of clinical depression, particularly if you are feeling down for a more extended period (more than two weeks) or if you’re finding it difficult to manage day-to-day life. Other symptoms to look for include:
- Insomnia or broken sleep and low level of energy
- Reduced self-esteem
- Changes in appetite
- Reduced libido
- A lack of motivation
- Emotional instability
- A lack of pleasure in activities which are generally of interest
- Difficulty concentrating
